What is the LLC name?

Is the LLC name registered with the county as a “fictitious business name”?

Identify your position in the LLC.

Is the LLC managed by all the members, or is there one or more designated “managers” (sometimes called the CEO)?

Identify the members of the LLC.

Identify the current manager(s) or CEO of the LLC.

Describe the general purpose and activities of the LLC and/or the types of goods and services that are provided.

Who or what business is listed as your registered agent?

If unsure, type "Not Sure".

Identify all insurance policies the LLC has, if any.

E.g. workers’ compensation insurance, general liability insurance, etc.

Does the LLC have a county business license?

Does the LLC have a seller’s permit?

Does the LLC use a resale certificate?

Has the LLC been filing its Statement of Information (SOI) biannually?

Does the LLC have an Operating Agreement?

Identify all amendments to the LLC’s Articles of Incorporation, Operating Agreement, or any other formation documents since incorporation.

Identify the member (ownership) split for the LLC.

Is the LLC currently working with a CPA and/or bookkeeper to help with its taxes?

Does the LLC have an EIN (Employer Identification Number)?

If the LLC has employees, how many?

Identify the employees’ names and corresponding positions.

Identify the LLC’s hiring process. (I.e. Does it have employment applications? Are interviews standardized or ad hoc? Etc.).

Does the LLC have any independent contractors?

If the LLC does have independent contractors, how many?

What types of independent contractors do the LLC have?

Identify all the human resource paperwork the LLC has in place for employees and/or independent contractors. (E.g. contracts, employee handbook, etc.).
