How many partners does the partnership have?

Classify which partner you are in the partnership, or identify if you are an Authorized Agent of the partnership.

Describe the general purpose and activities of the partnership and/or the types of goods and services that are provided.

Who or what business is listed as your registered agent?

If unsure, type "Not Sure".

Identify all insurance policies the partnership has, if any.

E.g. workers’ compensation insurance, general liability insurance, etc.

Does the partnership have a county business license?

Does the partnership have a seller’s permit?

Has anyone contributed money to the partnership by either a loan, investment, or a donation/gift?
